  • I think it's very easy for these people to make suggestions on flying the plane behind the safety of a computer screen. The truth is, the pilots had very little time to react, and under such extreme pressure, it's almost impossible.

  • There are a lot of comments about serile cockpits. I know a Monarch training captain who states that they stick religiously to their proceedure, but according to a BA training captain I know, BA are not so strict about it. I believe his words were "Not so sterile in my experience, nobody really pays attention to it. If you can't talk and fly a plane, you shouldn't be flying a plane"

  • @Neutrinoghost Sterile cockpit during critical flight situations (takeoff, landing, hard IFR, emergencies, problems, etc.) is a strict, hard & intnese policy of all airlines, but, as can be heard from this CVR, it is likely the unwritten policy of most pilots that a sterile cockpit is a bit silly. Like another said; "if you can't talk & fly, you shouldn't be flying." It's one of those oft-ignored rules like wearing safety shoes or hardhat. Needless until the rare time something heavy hits!

  • Something else that isn't talked about is this....They shut down #2 engine to save fuel due to a ground hold to Atlanta. A no flapper on 2 engines...unreal! Listen as you hear him call the loss of #2, they never had it.

  • There was another one like this, I think it was a MD-83. The pilots forgot to set the flaps all together and then they stalled and crashed into a bridge just after take-off. Except everybody but one person on the MD-83 died

  • @GPMSharpCommand That was a Northwest plane out of DTW back in the 80's. Only survivor was a little girl who was a lap child and was speculated to have been saved by being a lap child and being cusioned by her parent.

  • The findings from the NTSB for this crash are that because the captain had that laid back southern drawl he was an arrogant undisciplined dipshit american moron and failed to complete the checklist and killed everyone on board.

  • There was another Delta fight landing in Dallas in 1985 that stalled in a large storm system they tried to land but they ended up landing on the highway (killing 1 man on the ground) and came to rest at the airport after hiting a supply tank and tearing it in half 27 out of 130 some people died that day. R.I.P

  • This is one of the ultimate case-study accidents for making the case for sterile cockpit and checklists. These complacent, careless pilots killed people, and if they are still alive today, they know it.

  • I believe the entire flight crew of 3 on this plane survived the crash. Only 14 people on the plane died (2 flight attendants and 12 passengers). So I believe the people you hear screaming on the audio actually lived through the crash. Do a Wikipedia search for "Delta flight 1141" and you can read up on the whole thing.

  • @MrBkey No, this is real, Delta Air Flight 1411 in 1988. The recording's been widely distributed by the media. Besides, there are other CVR recordings out in the open on youtube, and I believe there's an album by the German band Rammstein that includes the last 40 seconds of Japan Airlines flight 123, the worst air crash involving a single airplane back in 1985.

  • Investigation reveals that the crew violated the "sterile cockpit" protocol; this distracted the crew from their duties of pre-takeoff preparation of the plane. The flaps were not set at all & the plane took off, but stalled immediately after. There was no remedy for this. All because they decided to discuss the dating habits of certain Delta employees. The FAA dictates there is to be no casual conversation between crew until above 10,000 feet for all FAR Part 91 flights.

Asshole, that was drinking and driving, crashed into him on a two lane highway. People that loose their relatives from other crashes, like these planes, or loose anyone that they care for, are real humans, and will be cared for more than people that think that this is a joke for their own amusement.

Here's the official cause: the Captain and First Officer's inadequate cockpit discipline which resulted in the flightcrew's attempting to takeoff without the wing flaps and slats properly configured," and listed as a contributing factor was the failure of the takeoff warning horn system. Also, had the captain followed proper stall procedure, the plane would have recovered and continued on its way, with only brown stains on the pilots' underwear.

  • @beeroosterm do you know the proper stall procedure? ease forward,full power. They were at most a few hundred ft above the ground does that seem like a good idea? I think not. So if the pilot did follow the proper procedure the impact would have been a lot worse and most would have died.

    Also they were taking of at the speed specified with flaps down when the take off speed required with no flaps is a lot higher, that plane was going down regardless of what the crew did. His actions saved lives

  • @nzAVIATORnz I would assume by your username that you are a pilot, as am I. The very first thing you are taught is the proper stall recovery procedure, which is, as you say, ease forward, full power. The trick is, when done properly, you can recover from a stall with little or no loss in altitude, especially if it's a nose-up stall. Too many times have I read NTSB reports where the pilot continued to pull on the yoke after the stall horn, which achieves nothing except maybe a deep stall.

  • @nzAVIATORnz It is better to retain control and recover from the stall than to let it stall into the ground. Had they done everything right (they practice V1 cuts for a reason) they would have got off the ground, around the pattern and on the ground again, without an accident.

  • If the pilots had adhered to the Sterile Cockpit Rule, paying attention to their pre flight checklist, making certain that the Flaps and Slats were properly set, instead of running their mouths about nonessential/irrelavant BS, those 14 people may still be alive today.

    Hopefully the flight crews certification was revoked, and the families of the deceased were successful in their wrongful death litigation against the negligent parties.

    Are you referring ot the co-pilot in this crash? The co-pilot on this aircraft was a guy in his mid-30's, IIRC.

    Luckily, most of the people on this aircraft survived. Prayers for the families of those who did not.

  • That screaming at the end isn't just the pilots. Sounds like the cockpit door banged open on the first impact and you're hearing the cries of 150+ people as the plane hit then bounced into the air again before the second, third, and fourth hits.

They crashed a perfectly good aircraft simply due to a complete disrespect for proper cockpit discipline and standard operating procedures. Read the FAA report and see what the cause was: The idiot pilots were so distracted by the fucking stewardesses they didn't set their flaps/slats to the takeoff position - something so basic that it's difficult to believe they forgot. In addition, the warning horn meant to go off if the flaps/slats weren't set properly was not functional; a perfect storm. Hope the look at those tits was worth it. 14 dead, 76 injured.

  • @garretts228 - Was a result of the pilots not properly configuring the plane for take off. This resulted in the plane not gaining the needed speed and striking runway equipment near the end.

  • The more you know about aviation, the more you realize that these scenarios play out over and over in one form or another. Proper CRM and sterile cockpit below 10,000 feet are relatively recent additions to the accident prevention arsenal. I don't think this particular tape is all that informative, if you think of it from the perspective of the people who perished, and the Captain who did'nt.

  • I see comments of people saying that the pilot forgot to set the flaps on takeoff, however, during take off, you hear the pilot say " We got engine failure" and a few seconds later they skid off 4000 feet past the runway. Would have to read the actual NTSB report but it seems to me its an engine failure during take off...

  • @Flyingfast79 In NTSB/AAR-89/04 report for probable cause, the only thing mentioned is improper procedure and the failure of the takeoff configuration system. When the main gear left the ground the plane rolled violently. Then there was a compressor surge. My thought on it is in a high angle of attack ( no flaps ) and in a stall, the air going into the rear mounted engines may have been disrupted due to turbulance created by the stalled wing. In the end it comes down to pilot error.

Most unfortunately, the pilot is not one of those killed. He has no one to blame but himself (perhaps the faulty warning horn would have woken him up, but I doubt it) If I were a family member of one of those killed, I would have made certain he suffered. Forgiveness is overrated...
