You dont need horizon. Just open a game save in resigner and click mod file and it has all of those options.

Hello alex , me and my friend have been trying to mod GTA 4 for about 3 weeks now , but we would like cars and different guns added to it . my computer that has ur modio (2.2 version) . everywhere that we go to look for the old modio it only comes with 3 or 4 beta . theres no way to mod GTA 4 and add cars in it . ive lost 3 accounts trying to mod this , please help !

@MayIUseThisUserName What you want is ISO Mods, they require a Dual-layer disc, a compatible burner, and a few other things. You cannot add any cars or guns to your game file by simply using a USB mod tool. In order to assign new cars/weapons you need to re-assign values given in the games original ISO, therefor, it's a lot more complicated and requires a whole disc to be used.
