Don't think its just that. Think its mostly about a guy in a doomed relationship with a woman he was losing. Part of the lyrics talks about losing her, the only one that knew him.

I think its a guy waiting for his girl to call after a fight when she dies. He asks God why but God responds by only showing up for him then, when hes at his lowest. God smoking his last cigeratte was letting him know that he let free will play thru as he waited & watched before finding the guy after all that time.
