I have to agree with RC about the cats added in, but this music score is one of about 10 that makes me believe all 8-bit music on Nintendo games is made this way.

Such a program exists. It's called notation player I think. If you load a MIDI file into it it shows where to place the notes. It's possible it was used, then again many good composers do it by ear.
