There were several points where McCain actually did look "lost", like when he held the mic is his lap, when he made that odd joke about the moderator not being chosen to be his Finacial Advisor, and also the "that one" comment.
I thought the same thing - that McCain did not look well. He looked tired and his speech was not always clear. I also found his comment (while pointing at Obama) "that" to really be in poor taste and very well could be looked at as a raciest comment (as if Obama was someone's property...) really bad taste.

Having been to a McCain townhall, I say it's just kind of his style. It's an unbecoming and downright awkward style, but a style all the same.
