Good job man, I'm learning bass and im sure this will come in handy, I originally heard the alkaline trio cover, but the cures original version blew my mind, keep up the good work.
I thought this was played on a six string base but sounds pretty good.

Chorus
G min chord - either G ang Bb on the 5th and 3rd frets of the D and G strings, or the 10th and 8th frets of the A and D strings
F chord - same idea; 3rd and 5th frets on the Dg and G strings for F and C, or 7th and 9th frets for the A and D strings
Eb chord- seee the above F but just move the notes down a whole step, and then back to the opening Bb chord!
Great job, but you seriously overthough this one. It's a pretty fun simple bass part

D min Chord- just play the open D string whil leaving your finger on the F on the G string
F Chord - either F and C on the 3rd and 5th frets on the D and G strings, or just drop to F and C on the 8th and 10th frets on the A and D strings
