There is a site called world of stuart that does a review of this excellent game and kindly links to a downoadable save file on mega upload with extra already unlocked for [people playing using r4 etc swap them over n us mere mortals can now enjoy extra without the frustration of death label. ps u must rename the save file to exactly what your current save file is named.
Death Label Doom is a real bitch... I finally managed to beat him, on my last life with zero bombs left. Seriously, he's Doom on steroids, but if you enter the fight with 5 or 6 lives left you should be able to scrape by. Good luck, because you'll need a fair share of that too.

Same deal with True Aki in Mushihimesama's Arrange mode; it's a cakewalk until that fag, but once you reach him, congrats, you've gotten as close to beating the game as any caucasian human being ever could.

Hmm... Aki on Arrange seemed MUCH harder than that ultra mode Aki. But I can't really say since I can only get up to stage 4 in Ultra. (Massive slowdown in PS2 version seems to help, I think)
I didn't get your first question. But the default lock-on shot is B (I think). You have to hit enemies with your thin laser first before you get your options to lock on.
As for extra mode, you can get it after you clear Death Label Mode.

Honestly, I don't have any no-miss-no-bomb Death Label replay. Except maybe for the first and second stage mid bosses but I didn't record them. And, I'm still working on score on Extra Mode

Honestly, the Extra Mode Doom is the real bastard. Got to him on last life with one bomb recently.
The attack sequence is really fast. But what really killed is the 3rd pattern on first form. Cut me clean. :))

It's alright.
Anyway, you have to hit the enemies with your "laser" first. By "laser", I mean the one the main ship fires.
Also, the closer you are to the target, the faster the options lock-on.
