This is humorous because we expect doctors to have a great deal of knowledge about medicine and to apply the knowledge honestly and accurately in order to keep their jobs, which the doctor in the video has not done.

The video is also fiction. Both of the people in the video are actors, not a real patient and doctor. No harmful deception is intended.

If I had a kid with down syndrome and I watched this video- yes, I would laugh. I'm not laughing at anyone, I'm laughing at this video because it's FUNNY. Not because I like making fun of people with down syndrome. My sister had cancer and it was shitty, but we all still make jokes about it. She lost her eye too; and pirate pick up lines are funnier than ever.
