This is Mafia: The City of Lost Heaven. The game was made in 2002, and I can't say that it has good graphics, but it is my favourite game. If you want to download the game, you must know, that it "weights" 1,5Gb.
Now about modding the game. To put any mod to game you must have "Mafia DataXTractor". Then extract needed folder from game(for example "models"). After that take modded files and replace original game files with them.
