A comment on the audio: Most of the videos from which this compilation was made had interesting audio tracks of their own. The guy who crashed the B-52 at Fairchild? Yes, unfortunately, your summary is about right. Tragically, in addition to killing himself, he killed other 3 other Air Force officers as well. If you watch closely, on his final go-around, you can see he nearly crashes on turn to downwind leg, but recovers. A few seconds later, on turn to final, he pulls the same stunt again, this time stalling and crashing. I'm beginning to wonder if maybe he committed suicide.

Failing to check free movement of control surfaces does, indeed, constitute "pilot error". Just as failing to check sufficient fuel (blame it on fuel guy?), failing to check for severe thunderstorms enroute (blame it on God?), failing to check proper weight balance (blame it on Sir Isaac?) etc are all actually pilot error.

yep. basically those models of Caribou had locks on the controlls (just like a steering wheel does). the pilot and ground crew failed to disable it, so when they pulled back it locked back, and resulted in that. quite sad really
It was an experimental Turboprop Caribou. I believe htre pilot's name was Perry. This belonged to NewCal/Pen turbo Aviation. The external aileron lock was in place. They are put in when you are parked in windy conditions to prevent the controls from slapping about in the wind.
