I think everyone missed the fact that the truck pulled out into two oncoming vehicles. The vehicle in the right oncoming lane nailed him for sure, possibly the vehicle in left lane as well. If the driver wasn't wearing a seat belt they were probably laying in the front passenger seat. The truck coasted into the train, it wasn't 'driven' into the train as if they didn't see it.

Here's what I see. A pickup truck makes a left turn into the road the train is crossing. The driver stops VERY close to the train. After a couple of uneventful seconds, a rail car which protrudes a little further than the rest strikes the pickup truck and sends it flying sideways. Rubber-necking drivers then bring the traffic on the nearby street to a standstill.
