They explain this in the manga. A person can exist in multiple dimensions, and each incarnation has a different role, but the person will still have the same relations wit the incarnations of the others in his/her life.

Simply put, TRC Sakura and Syoran and CCS Sakura and Syoran are different lifeforms, but the same in terms of their relationship.
