Just down the road where I livehere in the UK we have Felixstowe one of the biggest ports in Europe and to cope with the megaships such as the E Class Maersk ships Hutchinson Ports has had to spent £291 million on lengthen part of Felixstowe South to accomdate these giants and they are proposals for a new £300million terminal at Harwich as well to reduce the strain on Felixstowe but that has been held up.
these errors happen and they can't possibly be eliminated though it's best to reduce them, i think the errors would just be a misjudgment of the crane operator.
No big deal, an easy fix. Thank God there was no mistake in the position of the holes!!!! Moving the piece is a minor inconvenience. ( Relative to rebuilding it )

100 ton hatch lids? the most i've lifted are 34 ton and they are 5 or 6 boxes wide, and i mean 34,000 Kgs.
I think they like to exaggerate things a little.

Pay me 200,000 grand a year and I would work the cranes/drive the container trucks! Bugger me, the risk is minimal but the pay more than compensates.
