So here's my take on this.

The protagonist and the smiling man with white eyes are the same person. The white-eyed man represents the evil desires of the protagonist. At the cafe, he is fighting back his desire to rob the waitress, especially at the staredown at 1:03. He defeats the evil in himself that time. When he cheats on his wife, his dark side is winning, which is why he is shown smiling at him in the bar at 3:11. At the end, the protagonist does not die. He becomes the white-eyed man.
