This is just a base cake, you can make different sizes to make a 3 tiered cake and add baby stuff if you like.

By using a cake pan or cake ring and putting one diaper by one. When you got the rounded shape put a rubber band to keep them tied.

I bought the twisted paper in some crafts and papers stores in Argentina same as Mexico.Some people use it to do paper bows.
