It's not about stabbing your ear drum, Cotton buds can push ear wax further into the ear, where the cleaning mechanism cannot get to it.

Theres no need to ever clear out your ear, or stick anything into it. Your body cleans it out by its self.

I wouldn't reccomend Q-tips, for anywhere more than 5mm into the ear canal. That's why most people's ears get so dirty. It tends to push in the buildup more, and the cotton easily comes off.

I suggest you use an earpick, and if you're lucky enough to have someone willing to assist who has steady hands, take advantage of that and be thankful.
