Clannad has done weird things to me. I've spent most of my life watching overly masculine anime like Dragonball Z, Bleach, and other mainstream Shonen. When I first found Clannad, I didn't think I would like it, but it's the only show I've ever seen that's made me feel human. I could get a high of primal fury from watching impossibly powerful non-humans beat the crap out of each other for 300 episodes, but the love, sadness, and happiness of Clannad is a thousand times better.

They live many lifes. we follow Tomaoya through his last life, where he collects enough of the whishes to be transported to a second timeline where nagisa did not die. So most of the characters have died several times, and we dont know how many lifes tomoya has had, but we know that it is more than two.

TL DR: Everyone dies, several times. But we just got to witness some of it.

However, in the game, the "Main ending" was that Nagisa and Ushio died and that Tomoya passed out from his depression.

The "True Ending" is basically a little bit of extra for those who went 100% with the game.
