i've heard or read somwhere that the song is about so called Cristian Sience. Hetfield's parents belonged to that "church". And it's important that they disapprove of any kind of medicine - an that's why J's mother died when he was young. So his feelings towrds the Cristian Sience (which is somewhat connected to Scientism) are, understandably, negative. Hence the song is not satanist, but rather anti-Cristian Science and other institutions like it.
