Stu, you are right when you say this is a tragedy. How terrible it must be for all of their loved ones who saw mom or Dad going off to work as usual for this to happen. This was not even reported on the mainstream news channels here. So sad for all involved.

Yikes! Could this have been avoided by taking extra safety precautions? In the video you mention something about doors suddenly closing. Wouldn't it make sense to make it impossible to close the doors without diverting pressure first? I don't pretend to know anything about hydroelectric dam design, but something like that would make some kind of sense to me. It's like putting a railing at the edge of a tall cliff...
Their coaster gates on some units fail to close off the penstocks after the event overspeeding several units even with a completely flooded power house...
If the wicket gates close immediately stopping flow instantaneously the water hammer wont be stopped by the penstock/coaster gates... its too late...
My gut instinct says it was due to a design flaw much like Chernobyl, but I'm not one to dismiss random bad luck. It must be fortunate that the whole structure did not fail. Very scary!

There are much more risky industries but after 35 years in Nuclear, Geothermal, Combined Cycle Gas Turbine and Hydro I've seen my share of bad stuff...
It surprise me a tad as well... Mine is #7 in the world, this one is #6 largest... Our design is significantly better BUT similar accidents are vaugely possible here as well... We are built more robustly than they are as well...
Yes, well.. it still surprised me.. but I guess that how much we do think we have tamed nature, when let loose it still wrecks havoc.. and we can't really ever tame nature 100%.. it always has a way of showing who really is the boss..
Hydrodynamic forces are VERY powerful... this one got me in the gut thinking about my fellow Dam operators and their families, Russian or not it is a real tragedy for their people and their nation...

yes the entire occupants/workers at the time with 11 more missing I believe...
flooded the entire power house but down river flooding I don't believe so
An entire turbine was ejected with others overspeeding... A big design failure with possible operations errors...

They have a third or less the rebar in their concrete than here looking at the photos
Also They are much higher head design... consequently much greater water hammer in a wicket gate transient...
We are probably still suseptible to a head cover instance here worst case senario on the 6 big units...
