Prof Ancel Keys invented the lipid heart idea in the 1950s with his 6 Countries Study, but he had data from 22 countries. He ignored anything that didn't fit his idea.
What causes heart disease: tobacco, alcohol, stress, cocaine, sugar, corn syrup, grains and vegetable oils, except cold pressed veg oils.
There's no proof that lowering LDL in most people has any effect on heart disease. Statins, which do lower LDL, are beneficial for a small percentage of people, although there's no proof that it's because they lower LDL. Other agents which lower LDL have resulted in iincreased cardiac mortality which provide support for the idea that heart disease is caused by other processes.
