@MellonVegan No, this is a common thing in the US too. I think what David Letterman was trying to do was subtly point out that it is a weird thing to do when you're annoying by those situations. I think he obviously knows what it means but he often spends a good deal of time fixating on quirky habits (usually his own). After Paul describes what it means, Dave laughs, signaling that he already knew what it meant. Most people, if not all, know what that means here in the US.

Johnny Carson was hugely successful in the USA but bombed in the UK.
