How I Ink a Sketch

I noticed you ink directly over your sketches. How do you get/keep the lines so clean and crisp in photoshop? After I scan, every erase mark, pencil mark and etc can be seen. I also have trouble keeping the edges from looking jagged.

it seems like you use a lot of strokes, whenever i ink that way it starts to look sketchy, how do you keep the lines so smooth???
