Well that is what happens when you dnt tap. I'm a girl and I take bjj and I almost broke my sparring partners arm becuz he wouldn't tap. Me being new to bjj thought I applied the arm bar wrong when in fact he just wouldn't swallow his pride and tap. My sensei had to make him. So at times tap or snap. Megumi probably didn't think she applied enough pressure. It happens at times. Matt Hughes almost had to break Royce gracie's arm.
