The site will come up for you.

There is the actual form and the pdf explaining how to fill it in.

You can fill the form out online and then print it off.

You send the EX160 and the C1/C2 application for a hearing off together, with evidence of your benefit, that must be dated within the last 4 weeks.

Viola, that's it!! A £400 court hearing for zilch!!
