Poor arm/body on the backswing leading to a flat "top of the backswing" position. First move is over the top. Work on bringing the club more "up" and in front of your hands at the halfway back position. That should solve the problems. The over the top move is a compensation for the root of the problem: a poor, disconnected takeaway.
Nice athletic swing but I believe your arms and hands are out racing your body in the backswing probably causing a flat back swing or quick transition. In your backswing try starting by lowering your left shoulder as you turn. Your left shoulder should be 26 degrees (PGA pro average) lower than your right shoulder (ie your shoulder tilt) as you turn to maintain a spine angle that will create a good swing plane. Do the opposite on your follow through so you stay with the shot. Think tempo too.
Your lower body is moving to much causing you to lose balance. I highly recommend visiting shawn clements youtube page (clemshaw). He is a top 25 rated golfer who debunks much of the myths out there today and in terms easily understood. I went from 18 handicap to single digit in a matter of months. His videos can give you much more than people on here feeding you short term problem solving tips. Good luck!!
Not bad, but you are clearly going over the top with both clubs. I can easily see that your downswing comes over the top of your backswing club path. This is because your backswing is not upright at all. You should try crowding the ball a little bit more and get that left arm further up in the air on the way back.

You have a great swing, you just need to take your club straight back and more to the inside. So that and you'll improve a lot!
