I think that if we are to provide free healthcare to old people, the disabled, the military and children, then I think we should be able to provide healthcare to every US citizen. If we must practice socialism, then everyone should reak of its benefits. I don't think the majority should be coerced into paying the minority's healthcare. and I am not specifically talking about racial and ethnic minorities nor am I referring to whites as the majority.

As for the transition, I'd say some minarchist legislation would be nice, but I can't see that happening until an economic crisis hits or some catastrophe happens, and people realize it's the governments fault.
As for law, I think something like English Common law or Merchant would pop up, and the laws would be fleshed out by emergence. And as for the poor, perhaps those who were completely unable to work would receive charity, but we certainly wouldn't have all of the leeches we do now.
