It's a comment on how true can goodness be (Also based on a Cockney saying) when it is manufactured? An orange, sweet citrus on the outside, but on the inside, running mechanically on clockwork, because that is what he's been programmed to do.

Within the book (A Clockwork Orange, by Anthony Burgess) the old man from the movie is writing an essay about the new society and stuff. That was in fact what he was typing when Alex and co. broke into his house. The essay was supposed to be entitled...A CLOCKWORK ORANGE.

@Longwiew It was an old saying in Britain i believe. Anthony Burguess took the name after the expression "as queer (weird) as a clockwork orange". Hence the title.

It's about this Alex kid who goes around with his droogs doing the odd bit of ultra-violence. Gets caught, put into a correction facility. To say more would ruin it a bit. But it's about morality; whether an action is truly moral if we are forced into doing it.

When the cops hold Alex underwater that long, there was a breathing apparatus that Alex was supposed to be breathing on, but it malfunctioned. I forget where I read that. He wasn't that close to dying, but I imagine it was pretty horrible being held under like that.
