All I can recommend is to upgrade your computer. It'd help if you told us your system stats.

your graphics card maybe not good maybe try getting the nvidia geforce gt220 its cheep and good £60 it plays this game good. if your on a laptop then you cant do anything.

I have an 8800 GTS 640M and it looks great, I just cant get the opening cutscene to play correctly - looks grainy.

I am upgrading to a GTS 250, but in the meantime, whats a good setting to have. I have textures on high, and water and water GPU on, but just about everything set to off or med. any ideas

I have no idea why someone would say that. I bound the keys to a system that suited me and of course using a mouse to aim will always be better than a pad. I found the controls to be fine with mouse and keyboard.

The controls on the controller work very smoothly, except, maybe for the rapple when you first start using it.
