I like how the 'nightmare' mall isn't so visually intense. Whereas later in the game, at like...the amusement park, you've got the usual rust and grating everywhere.

Not necessarily. Heather is slightly out of reality during the whole game. Even if the world looks normal she is almost always the only human being in the area and things are blocked here and there.
