it doesnt say why john goes to the ER but it does say that it reflects badly on amy and ricky as parents

@buttons1492 They go to the ER because John's ear is hurting and Amy thinks he has a ear infection and when they go to the doctors they find out from the doctor what's really wrong and the problem is Ramy keep bickering with each other and John doesn't like it. So the doctor tells them they need to get a hold of themselves and stop fighting and John will stop holding his ears saying "ouch" when they yell at each other. And no Ricky and Amy don't break up in case you're wondering.

I honestly think the writers should just leave them together- they are the reason so many people watch the show. If they were smart that would continue like One Tree Hill did with Hayley and Nathan, by keeping them together and letting everything else change. They will struggle enough together as it is

supposedly its been months and not one day because "or not to be" was season 3 episode 26 and this is season 4 episode 1 so this is technically another season which was supposed to start some number of months after Monday's "season finale" but the next season started a week after the almost finale. :)
