Notice the man who says he has to push a pram. Remember, you carry babies in those. So why would he push it so much? He must have many babies to push. Lots of babies means lots of sex. Basically, he's boasting about how much sex he gets :).

A perambulator enables you to "walk" (or to push) a baby around, so the meaning of "I have to push the pram a lot" is that he is given that as a job, in other words, his wife sends him off out to take the baby for a walk, almost the exact opposite meaning to your interpretation
