I suppose the one comfort is that every time one of these disasters happen, air travel gets a little bit safer. Provided the airlines learn from it of course.
When will the plane's computer have Google Earth + GPS/radar and show the pilots the terrain far enough in advance to avert a crash? We have smart computers but not in these "state of the art" aircraft.
@normanjtongmd For all I know modern a/c have EGPWS for terrain awareness, and use IRS which is a stand alone system that doesn't rely on external factors (satellites), but they still also have GPS. Not an expert in any way, somebody correct me if I'm wrong
Pilots do watch these shows both to find out what the NTSB concluded and to learn from others mistakes. We all make them! Little things add up, like the man said it’s a chain of events not just one. It’s a shame that people have to lose a life before most things change. In the case of this particular crash the Controller was more at fault then the pilots. He first statement after acknowledgement of Flight 965’s contact to approach control should have been “Be advised we do not have Radar”.
@winterocks I agree that pilot errors are totally natural and they will always happen. The goal is to make airplane systems so that mistakes do not happen very easily, and if they happen it wont lead to disaster.
However I would not blame controller from this either.
But you sure can make a comment on everyone else’s abilities, like you know what you’re talking about. Oh I forgot your hiding behind a keyboard, where no one knows your name or identity. Most cowards do that don’t they?
What more upsetting is most of the dumb-ass and smart-ass comments are from people who are “Arm-chair Quarterbacks”. Not only are you not pilots, you wouldn’t have a clue on how to fly, what it takes to navigate day or night. I would grantee that if I took you up and had you close your eyes for three minutes then said take us back to the airport we just took off from, you’d be lost.
These careless and negligent pilots do not deserve to fly even a paper kite, let alone a Boeing 757, they made a silly and disastrous mistake that would not be expected from a PC flight simulator teen amateur!!
I know that there have been many tragic plane crashes, but this one hits me especially in a painful way. I'm not sure, but I think it may be because it could have been SO easily avoidable. But then, you could say that about other crashes...Perhaps it was the multiple mistakes made by the pilot, and the fact that had he just been a bit more careful, for example, looked at what he was punching into his computer screen, instead of just assuming that everything was going along as it should.
If all pilots watched all the Air Crash Investigation videos that show pilot error,, maybe once a year,, it might help them to stay aware of the very real dangers and stop them making the sort of mistakes that can end up in disaster. It also looks as if older more experienced Pilots need to be reminded that some very dangerous mistakes are made by highly experienced pilots,, and they are still prone to messing up. There's always the unexpected waiting to catch folk out once their guard is down.
So basically the whole thing was the fault of those bastard terrorists. If they hadn't blown up the radar the ATC guy could have warned the crew they were flying off course.
Air crash investigation?? less than a quarter of the item is actually about the investigation at all. The vast bulk seems to be filled with ghoulish and irrelevant personal details about the unfortunate victims
@Dougyman1983 I agree. Why tell the crew that they were cleared to Cali? They would think that they should delete all waypoints and head to Cali, which means they would take you literally.
They ALWAYS say "one of the most advanced airplanes in the world, with state of the art equipment". One day I want the commentator to say "They're flying a crappy airplane with a lousy navigation system, and these pilots got their training at Wal Mart". I'm NOT saying that's the case here, but just sayin the repetition gets old...
@nccsa186 You do know that Jonathan Aris is not being paid to make airline manufacturers look good,he's being paid for just the narrating. He reads from what is given to him.He may not agree with half if not most of the shit he reads.Lets point the blame to the proper people please.
Has anyone noticed, other than me, that a lot of these Air Crash Investigation and Mayday episodes involve American Airlines? I'm Canadian and I NEVER, NEVER will fly American Airlines. It seems that according to these series buying a ticket on American is like signing your own death certificate.
@ThatBearHasMoxie Yes but that was back then, now we have modern day tech so its really rare to have crashes. I flew AA and they gave excellent service
@ThatBearHasMoxie American Airlines is not any more dangerous than other airlines. It's simply cos of the fleet size of the world biggest airliners is so big, seemingly more accidents happen to them. Actually when you compare the number of accidents to performed flights, American Airlines is one of the safest airlines in the world.
But of course these accidents and incidents can ruin reputation of some airlines in peoples minds. I've never flown Air France cos I feel it somehow unsafe....
Aha, according to the Wikipedia article, "Colombia had duplicated the identifier for the Romeo NDB near Bogotá, and the computer's list of stored waypoints did not include the Rozo NDB as "R", but only under its full name "ROZO". In cases where a country allowed duplicate identifiers, it often listed them with the largest city first."
The controller did not understand proper phraseology. To go "direct" to a fix means you go directly there in a straight line without passing over any other fixes. But the Captain asked if he could go "direct Rozo" and the controller said "Affirmative" (this is on the CVR transcript). The controller later said that the Captain's request to fly "direct Rozo" did not make sense, so why did the controller answer "Affirmative"?
After the controller answered "Affirmative" the Captain attempted to fly direct Rozo, and he entered the identifier R from the chart, into the FMS. But for some bizarre reason the Columbia government used the same identifier for multiple fixes; the first near Bogota and the second near Cali. The computer, not being able to store two fixes with the same name, stored Rozo under ROZO instead of R. There was no way for the flight crew to have known this or to have expected it.
A court found the manufacturer of the FMS computer and the provider of its electronic navigation database a combined 40% at fault, and American Airlines 60% at fault. But I think that the role played by the Columbian government for giving two different fixes the same identifier, and the role played by the controller for allowing the Captain to think he was cleared DIRECT Rozo, are underappreciated.
@dmspilot From what I read in the report (in Spanish) the controller recognized that the flightcrew's request to initiate the approach at the penultimate waypoint did not make sense. He couldn;t express his doubts, so he reinstated the order (Take the Rozo 1) and in "Take the Rozo 1 approach procedure as it apprears on the charts.
@dmspilot Likewise he stated that he was aware that the flightcrew had gone past the Tulua VOR (since their final DME report was 36 miles or something along those lines, which was closer to the airport than the distance between the airport and the VOR) but he supposed (correctly at that) that the flightcrew simply forgot to report passing the waypoint. He knew something was not right but he couldn't point a finger at what it was.
Thats just sad..People who say im never flying on a plane again are stupid these kind of crashes are caused in the 1960's 1990's even 2000 but crashes that have been caused in the last few years are normaly survival ones hardly death ...
@damien131091 they put the wrong destination in the computer instead of just relying on their altimeter and a map. im not a pilot im a diver but theres one phrase that we go by plan your dive dive your plan. only one change can be made- abort. this relates to flying in the sense they plot their course on a standard map and fly the course like generations of pilots and captains before them instead of relying on a computer to tell them where they are
@tippman2k01 Exactly, they put in the wrong destination, that's human error. Its just like if you put a car in the wrong gear, you wouldn't blame the car!
And i don't think you can compare driving a car on a road with plenty of signs and information around you to flying a Commercial Jet 37,000ft in the air! They cant just use a map! They'd need some sort of checkpoint system.
How and why does it take 8 hrs to get the crash site? A whole military lacks night vision equipment? How do they even protect their own country? The little boy would have survived. Bure kabisa!

@AllSingingAllDancing , I guess make that list three!!!! I do it for some reason>.< I guess It makes me feel better as it makes me think that if theres a 1 in a million chances a plane would crash, then theres less chance for my flight to crash...
Hehe, I think it makes me feel better because now I know what the signs of something wrong with the plane look like, and if I don't see or feel anything, then everything'll be fine. Except on one flight I was on, there was some sort of indentation in the engine's covering, and I didn't know if that was supposed to be there so I was all "What if the engine disintegrates mid-flight? ; ;" And then there are the planes that get hit with other stuff. Double-edged sword.
@AllSingingAllDancing If the plane has WIFI or Internet on the In-Flight Entertainment you could watch this show on the plane. That would be even more twisted!

how hard is it to fly a plane?? its easier than packing at a grocery store, in grocery store u see the ticket and fill the shelves, in a plane u open the manual, go thru a routine check and fly.....and if it is about to crash, u open manual again and eventually crash
@1rubel yeah that's right, flying a plane is easier than going to the grocery store! (says the guy who has probably never flown a plane in his life!) Right, yeah and i am the queen of england, how do you do!
@win7sony No it's not. It's a card you fill in to speed up the process of entering the country upon reaching passport control on the ground! Information required upon entering the country is filled in on this card to make entry go as smooth as possible.
@hannahrenz It's a card you fill in to speed up the process of entering the country upon reaching passport control on the ground! Information required upon entering the country is filled in on this card to make entry go as smooth as possible.
They say that the computer was the pilot's worst enemy. I wouldn't say that because the computer did not do anything wrong... it were the clueless pilots who crashed the plane. When people cannot use the computer, then disaster strikes.

how hard is it to fly a plane?? its easier than packing at a grocery store, in grocery store u see the ticket and fill the shelves, in a plane u open the manual, go thru a routine check and fly.....and if it is about to crash, u open manual again and eventually crash

@win7sony No it's not. It's a card you fill in to speed up the process of entering the country upon reaching passport control on the ground! Information required upon entering the country is filled in on this card to make entry go as smooth as possible.

They say that the computer was the pilot's worst enemy. I wouldn't say that because the computer did not do anything wrong... it were the clueless pilots who crashed the plane. When people cannot use the computer, then disaster strikes.

Thankyou so much for uploading these - it's nice to have full episodes, and even nicer not having racist/ignorant comments slagging off everybody in the video.
