Queen Elizabeth II is Queen & Head of State of 16 independent countries across the world (notable ones include: the UK, Canada, Australia, New Zealand, etc). She is indeed literally Queen of Canada, and has various executive powers being Canada's head of state. In fact she is officially titled as 'Queen of Canada' under Canadian law.

Well, historically it would come from the fact that all of those 16 countries (except the UK itself and Papua New Guinea, which gained independence from Australia) were British colonies or collections of British colonies.
Eventually these countries gained independence. They could become republics, or constitutional monarchies. Many of the more successful countries, such as Canada, Australia, etc, which gained independence chose the Monarchy....
...and created distinct national monarchies, while keeping the same Queen, as head of state. They probably chose the system because it's quite successful and stable (according to the latest Democracy Index, 7 out of the top 10 democracies in the world are Constitutional monarchies). Currently Queen Elizabeth II is the only person to head more than one independent country in the world.
Yes God Save the Queen. The Queen of the Commonwealth, My Queen, the Queen of Canada, Our Queen. She is owned by us and is ours, as we are hers. Has there ever been such an equal pertnership in the history of the world? I ask this of all.

And of course, her children (in accordance with the Act of Succession) are heir to these thrones, as they are to the Throne of the UK.
And indeed, the Queen is really nice lady. Some member of the Royal Family have bad reputations, but the Queen has always been loved.
