ok i have almost the same CPU as you. I have Athlon 64 3800+ but it's single core BUT mine is 2,4 GHz so is mine more powerful than yours? even if your is with 2 cores.

Ive looked at some comparisons between the two, mostly showed the Radeon 4650HD being better than the 9500GT but not by much and it also showed that the 9500GT runs GTAIV better than the 4650HD, but other than that its about the same.
the "Effect" settings requires a higher CPU, disable it and leave all others on high, you shouldn't lag even a bit
