I might be totally wrong here, but what I get from the lyrics is this: despite Mika's confident stage persona, he is really still an insecure little boy at heart (a "one foot boy")

Basically I picture this all going through Mika's head right before he's about to go on stage. He's struggling with a lack of confidence, worried that he might have been"crazy all along". He can never get rid of these doubts no matter how hard he tries.

Anyway, whatever this song means I <3 it, it's so catchy!
