i know they lost the paper file that documented some acts of an abusive employee on me. I know my confidentiality was violated on several occaisons simply by a couple Msw's using the phone. Too many employees have access to records. Only the PHD and MD licensed med staff should have access. MSW's and other support staff should have only cursory info and they and others should have to make documented requests for specific info. Patient submissions are too often not accepted/kept too.
