Janitors also go through lots of training to have to do the really important part of their jobs too (that is, dealing with people and being hospitable: doctors could learn from janitors about this), it's just that this training is not recognized as institutional, since it's not as profitable to begin with (just being nice to people doesn't generate money to exploit).
