The budget was so low that they used real bums for the skid row scenes and when the dentist chair collapses at 3:15 in this scene instead of reshooting they just changed the script to read dentist chair collapses

The "dentist" is meant to be Seymour, which is why he's wearing raggedy clothing underneath the smock. The actual dentist is the "patient" who went "out the back door" that was laying dead on the exam chair.

Though I agree that Steve Martin and Bill Murray made the newer version a whole lot more fun...
