It looks like an acoustic tuner that works off the vibrations from the head. A good one is very accurate and has a fine tuning range.

Sometimes players put a large capo on the head which makes if accessible but also increases the sustain of the guitar.

I somehow was lucky enough to hear about thomas when Riddle came out.. what an album! he had just moved to Cali at the time, brilliant album, and one of a kind. I still have the original badly drawn riddle cover :)

Partly amplification, but also down to the ingenious way it uses more pull-offs than hammer-ons (which aren't limited in dynamic range like tapped hammer-ons). It also has the effect of sounding like there are more than three sustained parts. In theory it could probably be played even louder than the main theme.
