By itself, the expression "bless your heart" does not really mean "you're an idiot". What you would say if you wanted to say that someone is an idiot is "He's a pure-dee idjit, bless his heart." Basically the same thing goes for any insult. As long as you punctuate it with "bless your heart" (substitute the appropriate pronoun), you're OK.
