I pay my doctor out of pocket for regular office visits. He does it on a sliding scale and does not prescribe anything to me that is not covered by my prescription drug card from a private company. He also does not take medicare/medicaid - hence the sliding scale. HMO docs and docs who take medicare/medicaid wouldn't do that,. They charge everyone the same and prescribe the more expensive medications at full retail price determined by the manufacturers. HMOs usually have in-house pharmacies too
