If the ball and the throw get there at the same time - safe. If the throw beats the runner, he is out. Of course bang bang plays it is sometimes indistinguishable but at higher levels of baseball that is why the get paid the big bucks. Tough profession particularly for the guys who make it all the way through the minors into the majors.
