If this was recorded in February 2007, then this is not the first time everyone got the question wrong in Fastest Finger First in Who Wants To Be A Millionaire (WWTBAM).

The same thing happened in the German version, which was before February 2007, and exactly the same thing happened. Everyone got it wrong the first time, then all got it right the second time.

Search for "German WWTBAM: 2 Fastest Finger in a row" on YouTube, since URLs cannot be pasted.

Its also happened before then. In the australian version 2 contestants got the question right and had the same time. Then they went into a tie breaker and the 2 contestants both got the question wrong. That was in July 2000

We don't have any till now. So since the euro was introduced, 5 years ago, nobody has won the million. In the pre-euro version, from 1999 to 2001, one man won 1,000,000 gulden.
