At stores you can buy a card with a code to redeem TLAD with on the xbox marketplace or you can use microsoft points to buy it on the marketplace. Either way, you pay the same price (20$ or 1600MS Points). You can either buy points online on the xbox website, buy a card at stores, or i think buy them with a credit card on the marketplace.

This video was recorded while playing online, so all the players here have custom clothes. I just chose a jack that said The Lost on it when I customized my character. I don;t know if Johnny always wears the same stuff though, I heard you can buy more clothes but I don't know.
