Iran is a minor threat compared with the USSR and its allies during the Cold War, something Senator McCain ought to be aware of.
Israel is not part of the United States and is perfectly capable of defending itself with the hundreds of nuclear weapons it possesses. MAD would work in that case as in others even if Iran develops nuclear weapons itself, of which there is scant evidence.
