a) the refs are standing back the entire time while the linesmen break up fights

b) what fight did they stop early? all the fights ended when one guy fell onto the ice except the one where the players were tired and skated off themselves.

They couldn't. It's a suspension for removing your own helmet before the start of a fight. It looked like they both undid their own chin strap however, which made it easier for the helmet to come off.
