What makes the Activity Book Piggy's?

It is never mentioned in the episode who's Activity Book it is?

Pause it at 4:25: you will see Fozzie's teddy bear on the floor. Go to 7:43, and you will see that Fozzie's bear is now in the drawer

In other words, between 4:25-7:43, nobody put the bear away..

At 4:21, Kermit seems to jump out of nowhere from the above into the picture!

On the clothing page, his sailor suit he typicaly wears happens to be one of the clothing choices.

OK. This could be the 1989 intro, since I heard they shortened the opening that season. The 1990 intro combined clips from this and the 1984 opening. The one starting with them flying on a book might be from 1987.

Actually, the one with the book was from either 1986 or 1987, since it looks like Toei did the animation on that one. It looks like Akom did the animation for this short intro, so it might be from 1989, since they mainly started using the short theme at that time.
