She turns and fires at the sound because she knows what it was she heard - the cocking a gun. He's only about 4 feet away so you can test this if you like. Even a .38 revolver will be heard if it's cocked that close.

It's a psychological thriller, not an action movie. It's meant to have lots of dialogue, there was never going to be any shootouts or chase scenes, and if you honestly believe that this movie could be improved by the inclusion of a cheap, FX driven "good guy vs baddie" ending then clearly you are not cultured in the way of good films.
