i have the verizon iphone and when i ordered my iphone i ordered the otter box with it and my case fits perfect i didnt have the problem you described i would have taken back the case and tried another one because i think you may have just gotten a bad case

I ran to the same problem when I bought my otter defender case at Radio Shack, I forgot that the case was set up of ATT Iphone. I bought Verizon Iphone 32gb and Otter case defender. I struggle to lock in plastic part. Thanks now I know what to do to modify the case. It works now, mute button has space to slide top and bottom.
the same thing happened to me, i ordered mine on walmart.com but whatever i fixed it

@pioneer139 This case was purchased the day the iphone 4 came out for verizion. At that time the otterbox had the illustrated problem.
All cases sold now, like the one you bought, are designed to fit both models.
