You missed one. When you're in the scene with that small hole in the leprechaun chambers, wait a while and don't eat the mushroom.

When you're captured by the witch, you get held in the cage for a while before you die. Is there actually any way to escape?

A dwarf does appear on the stairs sometimes but he doesn't knock you off; he just steals some stuff and disappears.

Becasue the message is exactly the same as it is in the adjoining screens and since this was an early work, I didn't see any point in it at the time.
